Integrated Community Supports
At AXIS Care INC, our Integrated Community Supports (ICS) program is designed to empower individuals within our community to lead fulfilling lives by offering comprehensive and personalized support services. Services that provide support and training/habilitation in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting. We believe in fostering independence, nurturing strengths, and promoting holistic well-being through a range of tailored services aimed at addressing diverse needs.
Our Approach:
- Personalized Care Plans: Each individual is unique, and so are their needs. We craft personalized care plans by collaborating closely with clients, their families, and other stakeholders to understand their aspirations, challenges, and goals.
- Holistic Support: Our approach encompasses various facets of support, including but not limited to healthcare, social engagement, vocational training, and life skills development. We believe in nurturing the whole person.
- Community Integration: We facilitate seamless integration within the community by providing resources, guidance, and assistance to ensure individuals have the tools and opportunities to participate actively and meaningfully.
Eligibility
ICS is available to adults who live in an ICS setting and are on the following waivers:
- Brain Injury (BI).
- Community Access for Disability Inclusion (CADI).
- Community Alternative Care (CAC).
- Developmental Disabilities (DD).
Covered services
ICS covers training/habilitation and support to meet the person’s individualized assessed needs and goals in at least one of the community living service categories:
- Community participation.
- Health, safety and wellness.
- Household management.
- Adaptive skills.
