Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities and seniors find and keep housing. Housing stabilization services are state plan (not waiver) Home and Community-Based Services (HCBS).
Housing Consultation
We will develop a person-centered plan for people without case management services.
Housing transition services
Our housing transition services are individually tailored to support our clients find housing.
Housing Sustaining services
After developing a person-centered plan and finding housing, we help our clients maintain housing.
Eligibility
- Be on medical Assistance (MA)
- Be 18 years old
- Have a documented disability or
- Be experiencing housing instability
Our programs and services are custom-tailored to empower individuals with disabilities by providing comprehensive support and resources. We enable them to take charge of their lives and pursue their aspirations
- Support an individual transition into housing
- Increase long term stability in housing in the community
- Avoid future periods of homelessness or institutionalization
- Develop a Housing focus person centered plan base on assessment outcomes
- Develop a Housing transition
